This Privacy Policy explains how EvolvdRX ("we", "us") collects, uses, and shares information when you use our website and related services. Clinical services are provided by independent licensed providers through Locumtele, our affiliated clinical provider network. Protected health information (PHI) you share during intake, consultations, and ongoing care is handled by your treating providers as the HIPAA covered entities. Information we collect

• Information you provide: name, email, phone number, and other information you submit through forms or assessments.
• Account/assessment information: details you enter to receive services or recommendations.
• Medical information: health history, current medications, symptoms, lab results, and other information provided during your intake or telehealth visits. Information submitted through website assessments and consultation requests is processed by EvolvdRX and its service providers, including contact-management and request-processing services. Provider-hosted clinical intake is a separate step. See our HIPAA Notice for details.
• Payment information: processed securely through our payment processor. We do not store your full card number.
• Device & usage data: IP address, browser type, pages viewed, and similar data collected through cookies or comparable technologies.

How we use information

• Connect you with an independent, licensed healthcare provider for evaluation.
• Process your prescription and payment, and ship medication to your address.
• Communicate with you about your care, order status, billing, and account.
• Provide, maintain, and improve our website and services.
• Respond to inquiries and provide customer support.
• Maintain security, prevent fraud, and enforce our policies.
• Comply with legal, regulatory, and recordkeeping obligations.

How we share information

Your information is shared only with:

• Licensed healthcare providers through Locumtele who review your intake, make prescribing decisions, coordinate your care, and maintain your medical record.
• Licensed U.S. pharmacies that fulfill your prescription and ship your medication.
• Our payment processor for secure payment handling.
• Service providers who perform services on our behalf (such as hosting, analytics, and communications), under contract to use your information only for our purposes.
• Law enforcement or regulators, when required by law or to protect rights, safety, and security.

We use cookies and similar technologies to remember preferences and understand how the site is used.

Security

We use reasonable administrative, technical, and physical safeguards, including encryption, secure servers, and access controls, designed to protect information. Clinical information submitted through the provider-hosted patient platform is subject to the treating provider’s privacy practices. Website assessments and consultation requests are not a substitute for that clinical intake. However, no method of transmission or storage is 100% secure.

Your choices

• Access / correction / deletion: You may request access to, correction of, or deletion of your information by contacting us. Certain medical records may be retained as required by law and professional licensing rules, even if you request deletion.
• HIPAA rights: for requests involving your medical record (access, amendment, accounting of disclosures, restrictions), follow the procedures in our HIPAA Notice of Privacy Practices.
