Overview
This Consumer Health Data Privacy Policy explains how EvolvdRX ("EvolvdRX", "we", "us") collects, uses, shares and protects consumer health data when you use our website and related services. It supplements our general Privacy Policy and applies specifically to the category of information these laws call "consumer health data": personal information that is linked, or reasonably linkable, to you and that identifies your past, present, or future physical or mental health status.
It is written to satisfy the Washington My Health My Data Act (RCW 19.373), Nevada Senate Bill 370 (NRS 603A), and the consumer health data provisions the Connecticut Data Privacy Act added by amendment. Clinical care itself, and the protected health information (PHI) you share during an intake, assessment, or telehealth visit, is handled by your treating provider as the HIPAA covered entity; see our HIPAA Notice of Privacy Practices for how PHI is used and disclosed.
Consumer health data we collect
Depending on how you use EvolvdRX, consumer health data can include:
- Intake and assessment answers: the health history, symptoms, current medications, and treatment goals you share when you start a weight management, hormone, or sexual health assessment.
- Reproductive and sexual health information: anything you share about reproductive health or sexual health as part of a sexual health or hormone assessment.
- Prescription and treatment information: the medications a Locumtele-affiliated provider prescribes or recommends, and the dosing and treatment notes tied to your care.
- Biometric and bodily information: measurements such as weight, that you report as part of an assessment or ongoing care.
- Health-related browsing signals: which treatment or assessment pages you start or complete, where that reveals an attempt to seek a particular kind of health care.
Sources
- Directly from you: through intake forms, health assessments, consult requests, and messages you send us.
- From the clinicians who treat you: the Locumtele-affiliated licensed providers who review your intake and record your care create treatment and prescription information.
- From our licensed partner pharmacies: fulfillment and shipping status tied to a prescription.
- Automatically: through cookies and similar technology when you visit a EvolvdRX page tied to a health condition.
Why we use it
- To provide the product or service you asked for: connecting you with a Locumtele-affiliated provider, processing a prescription, and coordinating delivery through our licensed partner pharmacies.
- To run necessary internal operations: account administration, customer support, security, fraud prevention, and recordkeeping.
- To comply with legal and regulatory obligations.
Your rights
- Confirm: ask whether EvolvdRX is collecting, sharing, or selling your consumer health data.
- Access: request a copy of the consumer health data we hold about you.
- Withdraw consent: withdraw any consent you gave for the collection or sharing of your consumer health data at any time.
- Delete: request deletion of your consumer health data. When you ask us to delete, we also direct the service providers and affiliates who received it from us to delete it, as the law requires. Some records tied to your medical care may be retained where law or professional licensing rules require it, even after a deletion request.
How to exercise your rights
To exercise any of the rights above, contact EvolvdRX through our Contact page. Tell us which right you are exercising and, where asked, the information we need to verify your identity. We will not discriminate against you for exercising these rights, and we will respond within the time required by applicable law.

If EvolvdRX declines your request, you may appeal the decision by contacting us again through our Contact page and explaining why you believe the decision should be reconsidered. We will respond to your appeal within the time required by applicable law. If your appeal is denied, state law may let you take the matter further, including by contacting your state Attorney General's office.
